Complete Care at Madison is seeking an experienced Social Worker (LSW/CSW/LCSW) to join our skilled nursing and rehab team in New Jersey. This part-time position supports admissions, discharge planning, and family advocacy.

You will work within an interdisciplinary team to coordinate care, educate residents and families, and ensure documentation complies with state and federal regulations. Equal Opportunity Employer.

Responsibilities

• Coordinate admissions, discharges, and transition planning

Qualifications

• Current New Jersey LSW, CSW, or LCSW license required
• Minimum 1-2 years of experience in a healthcare setting, preferably skilled nursing, long-term care, or rehab
• Experience with discharge planning, Medicaid/Medicare, and MLTS preferred
• Understanding of CMS regulations and Department of Health survey process preferred
• Strong communication, documentation, and organizational skills
• Ability to work independently while collaborating with an interdisciplinary team
