Job summary

Complete Care at Woodlands is seeking an experienced and compassionate Social Worker (LSW / CSW / LCSW) to join our skilled nursing and rehab team.

This is a part-time role in a supportive environment where your voice matters and your experience is valued as you assist residents and families through admissions, discharge planning, and ongoing care coordination.

Qualifications

• Current New Jersey LSW, CSW, or LCSW license required.
• Minimum 1–2 years of experience in a healthcare setting, preferably skilled nursing, long-term care, or rehab.
• Experience with discharge planning, Medicaid/Medicare, and MLTS preferred.
• Understanding of CMS regulations and Department of Health survey process preferred.
• Strong communication, documentation, and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Coordinate admissions, discharges, and transition planning
• Provide emotional support and advocacy for residents and families
• Participate in interdisciplinary care plan meetings
• Educate residents and families regarding Advanced Directives
• Maintain accurate documentation in compliance with state and federal regulations
• Assist with discharge planning, community resources, and post-acute coordination
• Support residents through rehabilitation stays and long-term care placement
