Job summary

Saint Luke's South Hospital in Overland Park, KS is seeking a full-time Social Worker to join our team. Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning. Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner. Participates in program planning and development to meet the needs of customers within the entity, health system and community.* Shift: Full-Time Monday-Friday 8-4:40.
