Qualifications

• Master's Degree in Social Work is required.
• Hospital social work experience is preferred.

Responsibilities

• Assess the psychosocial, emotional, cultural and financial needs affecting health and recovery.
• Provide professional social work services with supportive counseling and clinical knowledge.
• Collaborate with the health care team to develop transition plans and address needs in a timely manner.
• Participate in program planning to meet patient and community needs.

The Opportunity

Responsible and accountable for assessing the psychological social emotional cultural and or financial needs that impact patients health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community. Help ensure our vision to be, "The Best Place to Get Care. Shift

PRN, 7:30 a.m. - 4 p.m. Minimum of 3 shifts per month, more available.

Education Required

Certified LMSW Missouri Social Work license Master's Degree in Social Work Hospital Social Work experience is preferred.
