Job summary

Saint Luke’s Health System in Kansas City, MO seeks a part-time Licensed Master Social Worker to provide social work services, including supportive counseling and clinical assessment of patients and families during illness, crises and discharge planning.

You will collaborate with the care team on transition plans, participate in daily treatment meetings, and deliver psychoeducational group sessions while arranging referrals and transportation as needed.

Qualifications

• Licensed Master Social Worker required (state-specific)
• Experience in psychosocial assessments preferred
• Crises intervention and discharge planning skills essential

Responsibilities

• Assess psychosocial, financial, cultural, and emotional needs impacting health and recovery.
• Provide supportive counseling and clinical social work services during illness and crises.
• Collaborate with health-care team to develop timely transition plans for patients and families.
• Participate in program planning to meet community and system needs.
• Conduct family therapy sessions and attend daily treatment team meetings.
• Deliver psychoeducational group therapy sessions to patients.
• Assist with discharge planning, referrals, and transportation arrangements

Job Description Responsible and accountable for assessing the psychological social emotional cultural or financial needs that impact patient s health and recovery.

To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning.

Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner.

Participates in program planning and development to meet the needs of customers within the health care system and community.

Completes family therapy sessions with patients and their families.

Participates in daily treatment team meetings.

Provides psychoeducational group therapy sessions to patients.

Assists with discharging planning, making referrals and assisting with transportation arrangements

Job Requirements Applicable Experience: Less than 1 year Licensed Masters Social Worker - Various Master's Degree
