Job summary

Saint Luke's Population Health - Physician Group seeks a Social Worker to assess psychosocial needs and provide counseling for patients and families. The role collaborates with the health care team to create transition plans and supports discharge planning.

Full‑time, Monday–Friday, 8‑hour days with flexible start times; coverage area includes multiple North Region primary care clinics in the Kansas City area. MO/KS LMSW or LCSW licensure is required; 1 year experience preferred.

Qualifications

• Excellent written and verbal communication skills.
• Healthcare setting experience preferred but not required.

Responsibilities

• Assess psychosocial, emotional, cultural, and financial needs impacting health.
• Provide professional social work services including counseling and assessment.
• Collaborate with care team to develop transition plans and timely responses.
• Participate in program planning to meet community and system needs.
• Work with patients/families across ages and diverse backgrounds.
• Maintain documentation and coordinate referrals as needed.

Job description

Saint Luke's Population Health - Physician Group is seeking a Social Worker to join our team! Responsible and accountable for assessing the psychological social emotional cultural or financial needs that impact patient s health and recovery.

To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning.

Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community.

Full‑time position working Monday through Friday, 8‑hour days, with flexible start time between 7:30‑8:30 a.m.

Coverage area is North Region Primary Care Clinics‑Barry Road, Shoal Creek, Smithville, and Platte City.

Services to patients and their families include:

• identification of psychosocial needs
• psychosocial assessment
• crisis intervention
• information and referrals
• protective services
• patient/family education

Must have the ability to work as part of a multidisciplinary team and with patients of various ages and cultural backgrounds.

Knowledge of theory and practice of social work at the MSW level and have basic research skills.

Must have excellent written/verbal communication skills.

Prefer prior experience in a healthcare setting (i.e., hospital, home health or nursing home) but not required.

MO and KS LMSW or LCSW required.

Job Requirements Applicable Experience: 1 year Licensed Masters Social Worker - Various Master's Degree - Social Work.
