Job summary

Saint Luke's Population Health - Physician Group is seeking a Social Worker to join our team. The role focuses on assessing psychosocial, cultural, and financial needs affecting patient health and recovery, delivering counseling, and coordinating discharge planning with the care team.

The ideal candidate holds MO/KS LMSW or LCSW, has a Master’s in Social Work, and can work within a multidisciplinary team to address diverse patient populations. Healthcare experience is preferred but not required.

Qualifications

• MSW degree with licensure (MO/KS LMSW or LCSW) required.
• Experience in healthcare settings preferred but not required.
• Excellent written and verbal communication skills.

Responsibilities

Saint Luke's Population Health - Physician Group is seeking a Social Worker to join our team!

Responsible and accountable for assessing the psychological social emotional cultural or financial needs that impact patient s health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community.

• Full-time position working Monday through Friday, 8-hour days, with flexible start time between 7:30-8:30 a.m.
• Coverage area is North Region Primary Care Clinics-Barry Road, Shoal Creek, Smithville, and Platte City.
• Services to patients and their families include: identification of psychosocial needs, psychosocial assessment, crisis intervention, information and referrals, protective services, and patient/family education
• Must have the ability to work as part of a multidisciplinary team and with patients of various ages and cultural backgrounds
• Knowledge of theory and practice of social work at the MSW level and have basic research skills
• Must have excellent written/verbal communication skills
• Prefer prior experience in a healthcare setting (i.e., hospital, home health or nursing home) but not required
• MO and KS LMSW or LCSW required
