Job summary

Autumn Lake Healthcare at Norwalk is hiring a Social Worker for a full-time role, Monday–Friday. The position emphasizes meetings with residents, families, and care teams to improve quality of life, with focus on building community referrals and supporting QA actions.

The ideal candidate will hold a valid Social Worker license and a Bachelor's degree in Social Work or a related field, with preferred prior experience as a Social Worker.

Qualifications

• Bachelor's degree from accredited school of Social Work or related field.
• Must have a Valid Social Worker License.
• Previous Experience as a Social Worker preferred.

Responsibilities

• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information so as to improve quality of life
• Establish relationships and maintain contact and referral flow with community based agencies/services that benefit our patient/resident population
• Assist the Quality Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies, prepare assigned information for input in to the MDS computer program
