Job summary

Autumn Lake Healthcare at Linwood is seeking a compassionate Social Worker to join our caring team. This role focuses on resident well-being, discharge planning, and coordinating with families and care teams to ensure smooth transitions.

Candidates with valid Social Worker license and two years of geriatric healthcare experience are encouraged to apply. MSW/CSW preferred; strong communication and computer literacy are essential for success in this position.

Qualifications

• Previous experience as a social worker preferred.
• Valid Social Worker License.
• Two years experience working directly with geriatric clients in a health care setting, desirable
• MSW, CSW preferred
• Customer service, in-person and over the telephone
• Some knowledge of medical insurance, Medicare/Medicaid program benefits, desirable
• Discharge planning
• Computer literacy, able to fax, scan, email
• Experience in clinical patient conferences.
