Job summary

Autumn Lake Healthcare at Summit Park is seeking a Licensed Social Worker to join our compassionate team. The role focuses on psychosocial assessments, care planning, and advocacy for residents aging in place.

Responsibilities include attending care plans, MOLST reviews, and collaborating with the interdisciplinary team to complete discharge planning and MDS items. Knowledge of directives and resources for residents is essential.

Qualifications

• Psychosocial assessments using BIMS/PHQ9/Trauma Informed Care.
• Attend care plans and meetings to align with resident goals.
• MOLST review as part of care planning.
• Knowledge of HCPOA/Advanced Directives, PASSAR/ID screens.
• Collaborate with Interdisciplinary team on discharge planning, MDS completion, and individualized care plans.
• Provide resources PRN to residents and families.
• Advocate for seniors aging in place and quality of life.
• Attend meetings as appropriate.

Responsibilities

• Conduct psychosocial assessments and document findings for care planning.
• Participate in care plan meetings and quality discussions.
• Perform MOLST reviews and ensure directives are up-to-date.
• Know HCPOA/Advanced Directives procedures and PASSAR/ID screens.
• Coordinate discharge planning with the care team and MDS tasks.
• Offer available resources to residents and families as needed.
• Advocate for aging in place and residents’ preferences.
• Attend required meetings to ensure continuity of care.
