Job summary

Autumn Lake Healthcare in Linwood, New Jersey, is seeking a compassionate Social Worker to join our team. You will provide psychosocial support to residents, assist families, coordinate discharge planning, and collaborate with an interdisciplinary care team to optimize quality of life for geriatric patients.

Qualifications

• Previous experience as a social worker preferred.
• Valid Social Worker License.
• MSW or CSW preferred.
• Two years experience working directly with geriatric clients in a health care setting desirable
• Customer service, in-person and over the telephone
• Some knowledge of medical insurance, Medicare/Medicaid program benefits desirable
• Discharge planning
• Computer literacy, able to fax, scan, email
• Experience in clinical patient conferences
