Job summary

Autumn Lake Healthcare at Summit Park is seeking a Licensed Social Worker to join our team, providing compassionate, patient-centered social work support. Our team emphasizes collaboration with residents and families to optimize discharge planning, resource coordination, and advocacy for seniors aging in place.

Candidates should hold a valid Social Worker License and have experience in psychosocial assessments and care planning within a multidisciplinary setting.

Qualifications

• Experience as a Social Worker preferred.
• Requires a Social Worker License.

Responsibilities

• Conduct psychosocial assessments (BIMS/PHQ9/Trauma Informed Care).
• Attend care plans and meetings.
• Perform MOLST reviews.
• Knowledge of HCPOA/Advanced Directives, PASSAR/ID screens.
• Collaborate with Interdisciplinary team on discharge planning, MDS completion, and individualized care plans.
• Provide resources PRN.
• Advocate for seniors aging in place.
• Attend meetings as appropriate.
