Job summary

Atrium Center for Rehabilitation and Nursing in Brooklyn, NY is seeking a Social Worker to provide essential support, guidance, and advocacy for residents and their families. The position is full-time with a daytime schedule.

You will interview residents, collaborate with families, assist with discharge planning, and coordinate community resources, working with an interdisciplinary team to improve residents' quality of life.

Qualifications

• MSW preferred.
• Bachelor’s degree required in Social Work, Sociology, Psychology, Special Education, Rehabilitation Counseling, or a related field.
• Minimum of one year of supervised social work experience in a healthcare setting.

Responsibilities

• Conduct resident and family interviews to develop comprehensive social histories and individualized care plans.
• Partner with families to create programs that support residents’ emotional, social, and psychological well-being.
• Assist with discharge planning, financial resources, and coordination of community support services.
• Collaborate with an interdisciplinary team—including nursing, administration, and medical staff—to ensure holistic care.
• Advocate for residents by addressing social and emotional needs and maintaining clear, effective communication.

Salary: $60,000 – $65,000 annually

Key Responsibilities

Qualifications

• Master’s Degree in Social Work (MSW) preferred

• Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals

• Experience in a Skilled Nursing Facility (SNF) strongly preferred

• Strong interpersonal and communication skills with the ability to build trust and rapport
