Job summary

JGS – Leavitt Family Jewish Home in Massachusetts is seeking a Social Worker to assess residents’ psychosocial needs and develop personalized care plans to support discharge planning based on experience.

The role offers $30.00-$34.00/hour, a $3,000 sign-on bonus, full-time employment, and a team-focused environment that values resident rights and employee appreciation.

Qualifications

• Bachelor or Master of Social Work degree (BSW/MSW) required or preferred.
• Current MA Social Worker license (LSW) in good standing.
• Prior experience in a long-term care setting strongly preferred.
• Ability to communicate fluently in English, both verbally and in writing.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized care plans for residents.
• Provide counseling, intervention, and ongoing support to residents and families, advocating for resident rights and quality of life.
• Collaborate with an interdisciplinary care team to coordinate services and facilitate care conferences.
• Connect residents and families with community resources and post-discharge support for smooth transitions.
