Job summary

JGS – Leavitt Family Jewish Home in Massachusetts is seeking a Social Worker to join our care team. The role focuses on psychosocial assessments, care planning, counseling residents and families, and coordinating with an interdisciplinary team to ensure resident-centered care.

The ideal candidate holds a BSW or MSW, an active MA Social Work license (LSW), and prefers long-term care experience; fluent English is required.

Qualifications

• Bachelor or MSW in Social Work (BSW/MSW).
• Current MA Social Work license (LSW).
• Long-term care experience strongly preferred.
• Fluent English communication (verbal and written).

Social Worker | JGS – Leavitt Family Jewish Home — $30.00 - $34.00/hour, based on experience • $3,000 Sign-On Bonus • Full-Time

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs.
• Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life.
• Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.
• Connect residents and families with community resources and post-discharge support services to promote successful care transitions.

Qualifications

• Bachelor or Master of Social Work (BSW or MSW)
• Current, unencumbered MA Social Work license (LSW)
• Prior experience in a long-term care setting strongly preferred
• Ability to communicate fluently in English – both verbally and written
