Moravian Manor Communities in Lititz, PA, is seeking an experienced Social Worker to join our Social Services team. This full-time role offers 40 hours per week, Monday through Friday, with competitive compensation and comprehensive benefits.

Qualifications

• BS in Social Work from an approved program or a related BS in human services (Sociology/Psychology).

• 3-5 years of experience working with seniors.
• Basic knowledge of Windows and Office software; willingness to learn department-specific training.

Responsibilities

• Provide Social Services support for residents of Moravian Manor Communities.
• Complete initial social service paperwork (social histories, discharge plans, needs assessment, supportive services, care transitions).
• Assist with Medical Assistance process for services.
• Conduct needs assessment interviews and maintain records of supportive services.
• Monitor residents for changes requiring support or care transitions; coordinate transfers between levels of living.
• Plan and implement daily/weekly/monthly events to encourage socialization and meet resident needs.

Moravian Manor Communities, located in the charming town of Lititz, PA, is a respected not-for-profit senior living community with a 50-year tradition of serving older adults. We are currently seeking an experienced Social Worker to join our dedicated Social Services team and make a meaningful impact in the lives of those we serve.

This full-time opportunity offers competitive compensation, comprehensive benefits, and a consistent Monday through Friday schedule, working 40 hours per week from 8:00 a.m. to 4:30 p.m.

Responsibilities of a Social Worker:

• Provide Social Services support for residents of Saxony Ridge Apartment Building and Moravian Manor Communities Residential Living.
• Assists with completion of initial social service paperwork, including social histories, discharge plans, needs assessment, and identification of supportive services and care transitions.
• Assist with the Medical Assistance process for services.
• Completes needs assessment interviews, enters information into records, and updates need for supportive services.
• Maintains awareness of changes in residents that may necessitate a need for support services, or a move to the next level of care. Assists social services and other Moravian Manor staff in coordination of transfers to and from other levels of living to or from residential living, if applicable.
• Plans and implements daily, weekly, and monthly events and activities to encourage socialization and recreation that meet the needs and requirements of residents at Moravian Manor Communities, and meets regularly with the Director of Community Life and/or Community Life Coordinator to coordinate and implement all activities/events.

Requirements and Qualifications of a Social Worker:

• Must have a BS in Social Work from a school approved by the Council in Social Work Education of a BS in human services field such as Sociology or Pyschology.
• Knowledge and understanding of aging population.
• 3-5 years' experience working with seniors
• Must have basic knowledge of Microsoft Windows and Office software and have the ability to adapt to new software and be willing to continue with additional department-specific computer training.
