Elm Terrace Gardens in Lansdale, PA is seeking a compassionate Social Worker to support residents and families in our skilled nursing program. You will conduct psychosocial assessments, develop individualized care plans, provide emotional support, coordinate with community resources, and advocate for residents’ rights and smooth transitions.

Qualifications

• Bachelor’s degree in Social Work (BSW) or related field; Master’s degree preferred but not required.
• Valid state licensure or certification as an LSW or equivalent.
• Experience in healthcare or skilled nursing environment preferred.
• Strong interpersonal and communication skills with diverse populations.
• Knowledge of healthcare regulations, Medicaid/Medicare policies, and community resources.
• Empathy, professionalism, discretion and excellent documentation.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities
