Neffsville Nursing & Rehab is seeking a full-time Social Worker to join our compassionate team in Lancaster, PA. In this role, you will serve as a key advocate for residents—supporting their emotional well‑being, guiding families, and helping ensure a smooth transition into and throughout their stay with us.
Complete social assessments and care plans addressing residents’ psychosocial needs with realistic, actionable goals
Participate actively in pre‑admission and admission processes
Provide compassionate support to residents and families during adjustment, crisis, or periods of heightened need
Document behavioral and psychosocial updates in accordance with facility and regulatory requirements
Lead individual and group support as needed
Develop and grow volunteer programs tailored to resident needs
Orient new residents and families to the center to ensure a smooth transition
Bachelor’s degree in Social Work or related field preferred
Knowledge of long‑term care social services practices, regulations, and guidelines
Strong communication, empathy, and independent problem‑solving skills
