Job summary

Fort Lauderdale Health & Rehabilitation Center is seeking a dedicated Social Worker to support residents’ social and emotional well-being in a Skilled Nursing Facility setting.

The role involves coordinating discharge plans, conducting resident interviews, and collaborating with community resources to ensure safe transitions. Prior Medicaid exposure in LTC/SNF is preferred, with a focus on integrating new social services and approaches.

Qualifications

• Minimum of 1 year social work experience in a health-care setting.

As a Social Worker, you will play a vital role in ensuring the social and emotional well-being of our residents.

About the Role

As a Social Worker, you will assist in planning, organizing, implementing, evaluating, and directing the overall operation of our Social Services Department in accordance with current federal, state, and local guidelines and regulations. Your responsibilities will include:

• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Interview residents and families to obtain social history.
• Work with community resources to ensure the patient has a safe plan when they transition out of the facility.

Requirements
