Job summary

Southern HealthCare Management in Florida seeks a Social Services Assistant to support the social services function in a nursing center. You will work with the Social Services Manager to admit new residents, complete psychosocial assessments, care plans, and discharge plans, ensuring a comfortable and positive experience while supporting treatment goals.

This full-time role offers a supportive, team-focused environment with dayshift hours from Monday through Friday, promoting a stable work

Qualifications

• Bachelor’s degree in social work or related human services field.
• One year supervised social work experience in a health care setting.
• Proficient in medical terminology and computer use.

Responsibilities

• Welcome and admit new residents and patients.
• Complete psychosocial assessments, care plans and discharge plans.
• Support the Social Services Manager to ensure a positive patient experience.

You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. You'll enjoy a Monday - Friday, dayshift work schedule.

Qualifications

• Minimum high school diploma or equivalent required.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• One or more years’ experience in health care education/ marketing / insurance or commensurate educations preferred.
• Must be familiar with medical terminology.
• Proficient in the use of personal computer.
