Job summary

National Health Care Associates seeks a dedicated Social Worker to advocate for residents and ensure holistic care in a Bloomfield long-term care setting.

You will conduct psychosocial assessments, coordinate with families and the care team, and provide counseling and crisis support to residents and their loved ones. A strong commitment to person-centered care is essential.

Qualifications

• Master’s degree in social work from an accredited program.
• Current licensure or certification as a Social Worker in the state of employment.
• Experience in Long-Term Care, geriatrics, or related field preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs.
• Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans.

Key Responsibilities Of a Social Worker

• Conduct comprehensive psychosocial assessments to identify residents\' social, emotional, and environmental needs, as well as strengths and preferences
• Collaborate with residents, their families, and the interdisciplinary team to develop individualized care plans that address social, emotional, and psychological concerns
• Provide counseling and emotional support to residents and their families, offering guidance on coping strategies, adjustment to care transitions, and decision-making processes
• Serve as a strong advocate for residents\' rights and preferences, ensuring their voices are heard and respected in care planning and decision-making processes
• Connect residents and their families with community resources, support services, and advocacy organizations to address social, financial, and legal needs
• Provide crisis intervention and support during challenging situations, such as grief and loss, family conflicts, or changes in health status

What You\'ll Bring

Qualifications of a Social Worker include:

• Master’s degree in social work from an accredited program
• Current licensure or certification as a Social Worker in the state of employment
• Previous experience in Long-Term Care, geriatrics, or a related field
• Strong assessment, counseling, and crisis intervention skills
• Excellent communication and interpersonal abilities
• Ability to work collaboratively as part of a multidisciplinary team
