Job summary

Hebrew Center for Health & Rehabilitation in West Hartford is seeking a Weekend Social Worker/Admissions. As a Social Worker, you will advocate for residents' well-being and ensure holistic needs are met, playing a pivotal role in improving quality of life.

Key responsibilities include psychosocial assessments, care planning with families and the interdisciplinary team, counseling, advocacy for residents' rights, crisis support, and linking families to community resources.

Qualifications

  • Master’s degree in social work from an accredited program.
  • Current licensure or certification as a Social Worker in the state of employment.
  • Experience in Long-Term Care, geriatrics, or a related field.
  • Strong assessment, counseling, and crisis intervention skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Empathy, compassion, and a commitment to person-centered care.

Responsibilities

  • Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs, as well as strengths and preferences
  • Collaborate with residents, families, and interdisciplinary team to develop individualized care plans that address social, emotional, and psychological concerns
  • Provide counseling and emotional support to residents and families, offering guidance on coping strategies, adjustment to care transitions, and decision-making processes
  • Serve as a strong advocate for residents' rights and preferences, ensuring their voices are heard and respected in care planning and decision-making processes
  • Connect residents and their families with community resources, support services, and advocacy organizations to address social, financial, and legal needs
  • Provide crisis intervention and support during challenging situations, such as grief and loss, family conflicts, or changes in health status

Weekend Social Worker/ Admissions
What You'll Do:
