Documents all interactions with patients and families in the electronic medical record.45%Facilitates the preadmission and admission process with the patient’s family.10%Acts as a liaison between interdisciplinary team and patient/ family by facilitating communication, answering questions, and offering professional guidance during the inpatient stay.25%Ensures a safe discharge plan by collaboration with the patient/ family and various disciplines to address all aspects of the patient’s needs upon discharge.10%Follows the patient and family after discharge, ensuring satisfaction and continued support.5%Other Duties as assigned5%Education:Bachelor’s Degree required.
