Job summary

Bethany Children’s Health Center in Bethany, Oklahoma seeks a Social Worker to provide emotional support, crisis intervention, advocacy and bereavement services to patients and families during hospitalization.

Responsibilities include documenting in EMR, guiding preadmission and admission processes, coordinating with the interdisciplinary team, ensuring a safe discharge plan, and providing follow-up support after discharge.

Qualifications

  • Bachelor’s Degree required; Master’s Degree preferred.
  • LMSW or LCSW preferred.

Essential to such support is the appropriate and positive interaction between the child’s caregivers and the staff.Essential Duties & ResponsibilitiesDutyPercent of TimeThe Social Worker is a primary resource and contact for the patient and family throughout the patient’s stay by providing emotional support, crisis intervention, advocacy, and bereavement services. Documents all interactions with patients and families in the electronic medical record.45%Facilitates the preadmission and admission process with the patient’s family.10%Acts as a liaison between interdisciplinary team and patient/ family by facilitating communication, answering questions, and offering professional guidance during the inpatient stay.25%Ensures a safe discharge plan by collaboration with the patient/ family and various disciplines to address all aspects of the patient’s needs upon discharge.10%Follows the patient and family after discharge, ensuring satisfaction and continued support.5%Other Duties as assigned5%Education:Bachelor’s Degree required.
