Job summary

Laurels of Carson City in Carson City, MI is seeking a dedicated Social Worker to provide psychosocial support to residents and families. The role collaborates with the interdisciplinary team to address emotional, social, and care-transition needs while ensuring compliance with federal and state regulations.

Qualifications

• Bachelor's degree in social work or human services field required.
• One year supervised social work experience in health care.
• Working knowledge of federal and state long-term care regulations.
• Knowledge of chronic disease management and care transitions preferred.

Responsibilities

• Supervises psychosocial support and services provided by Social Worker and Social Services Designee.
• Carries out supervisory responsibilities in accordance with the organizational policies and applicable laws.
• Interviews, recommend hires and trains employees
• Plans, assigns, and directs workflow.
• Prepares work assignment sheets, taking into consideration resident safety, the educational preparation, experience, knowledge and ability of the persons to whom the assignments are made.
• Receive calls and find replacements for absent staff as needed
• Transfers employees within units as needed.
• Prepares performance evaluations with the understanding that such evaluations impact tenure of probationary employees and wage increases of non-probationary employees.
• Schedules and adjusts lunch and rest breaks.
• Receives and resolves employee complaints.
• Monitors and corrects job performance of employees.
• Adequately supervises assigned staff and assures that those staff are supervising employees assigned to them.
