Job summary

Ciena Healthcare in Michigan is seeking a Social Worker to provide psychosocial support to residents and families, ensuring compliance with regulations and supporting care planning, discharge planning, PASARR compliance, and quality of life initiatives.

The Social Worker collaborates with the interdisciplinary team to address emotional, social, behavioral, and environmental factors that impact resident care and transitions.

Qualifications

• Bachelor's degree in social work or related field required.
• One year supervised social work experience in health care setting preferred.
• Knowledge of federal and state long-term care regulations beneficial.

Responsibilities

• Supervise psychosocial support and services provided by Social Worker and Social Services Designee.
• Carry out supervisory responsibilities in accordance with organizational policies and applicable laws.
• Interviews, recommend hires and trains employees.
• Plans, assigns, and directs workflow.
• Prepares work assignment sheets with resident safety and staff qualifications in mind.
• Receive calls and arrange replacements for absent staff as needed.
• Transfers employees within units as needed.
• Prepares performance evaluations affecting tenure and wage increases.
• Schedules and adjusts lunch and rest breaks.
• Receives and resolves employee complaints.
• Monitors and corrects job performance of employees.

• Prepares performance evaluations with the understanding that such evaluations impact tenure of probationary employees and wage increases of non-probationary employees.
• Schedules and adjusts lunch and rest breaks.
• Receives and resolves employee complaints.
• Monitors and corrects job performance of employees.
• Adequately supervises assigned staff and assures that those staff are supervising employees assigned to them.

Education and/or Experience:

• Minimum of bachelor's degree in social work or bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
• One year supervised social work experience in a health care setting.
• Working knowledge of federal and state long-term care regulations.
• Knowledge of chronic disease management and care transitions/discharge planning is preferred.

Certificates, Licenses, Registrations:

• Current Social Work license in the state as required by state regulations.
• LSW, LMSW, LCSW, LBSW, LLBSW, or LLMSW preferred.
