Qualifications

• Bachelor's degree in social work or related human services field.
• One year supervised social work experience in a health care setting.
• Working knowledge of federal and state long-term care regulations.
• Knowledge of chronic disease management and care transitions/discharge planning preferred.

Responsibilities

• Supervises psychosocial support and services provided by Social Worker and Social Services Designee.
• Interviews, recommend hires and trains employees.
• Plans, assigns, and directs workflow.
• Prepares work assignment sheets with safety and qualifications considerations.
• Transfers employees within units as needed.
• Prepares performance evaluations affecting tenure and wage increases.
• Schedules and adjusts breaks.
• Receives and resolves employee complaints.
• Monitors and corrects job performance of employees.
• Adequately supervises assigned staff and ensures others are supervised.

Education

Bachelor's degree in social work or bachelor’s degree in a human services field
