Job summary

Friendsofkerhonkson seeks a Master’s Social Worker (MSW) licensed in Michigan to assess and coordinate social services for participants and families. You will work with the IDT to develop care plans, provide CBT/Motivational Interviewing counseling, and manage case coordination from enrollment through discharge.

The role requires strong knowledge of EHR systems, team collaboration, and adherence to local/state/federal regulations.

Qualifications

• Must hold a Master’s in Social Work and be licensed to practice in Michigan.
• One year of clinical social work experience, preferably in geriatrics.
• Demonstrated knowledge of case management, short-term interventions, and patient-centered care.

Responsibilities

• Conduct comprehensive bio-psychosocial assessments at enrollment and at 180 days.
• Develop and implement individualized care plans for participants and caregivers.
• Provide counseling using CBT and Motivational Interviewing; educate and support families.
