Job summary

Responsibilities

• Provides psychosocial assessment of factors related to the resident’s illness and need for care.
• Identify and provide problem resolution associated with high risk indicators endangering resident’s mental and physical health.
• Identify support system.
• Evaluate resident(s) and implement treatment plan.
• Consults with residents as needed including provision of counseling and direct social worker services to assist with meeting psychological needs of resident/family.
