Formación

• Bachelor’s Degree in Social Work required.
• CPR certification required.

Responsabilidades

• Supervise Social Work Department and Activities Coordinators/Assistants.
• Conduct psychosocial assessments and develop treatment plans.
• Coordinate discharge planning and transition options for residents/families.
• Maintain accurate documentation in residents’ medical records.
• Communicate goals and plans of care to residents, families, staff and physicians.
