Qualifications

• Master's degree from an accredited school of social work.
• LCSW licensure by the State of NJ.
• NPI and Medical Non-Billing Provider Number required; apply within 30 days.
• Clinical Supervision Certificate required to supervise LSW or within 1 year of hire.
• Certified EARC Trainer-Department of Health and Senior Services or within six months of hire.
• At least five years of social work and discharge planning experience; acute care hospital preferred.
• Managerial experience preferred.
• Knowledgeable in psychosocial assessment, discharge planning/transitions in care, and related regulations.
• Excellent written and oral communication and strong planning and organizational skills.
• Strong critical thinking and analytical abilities for complex patient cases.

Responsibilities

• Manage daily operations and supervision of Social Work staff.
• Integrate Social Work services into departmental and organizational care systems.
• Oversee discharge planning, transitions in care, and reimbursement considerations.
• Manage hospital ambulance transportation contract operations.
• Lead behavioral health and ethical issues; collaborate with Care Coordination Nursing Manager.
