ChristianaCare in the Mid-Atlantic invites a skilled social worker to join our Day shift team. You will provide psychosocial assessments, discharge planning, and collaborate with a multidisciplinary team to support patients and families during illness and recovery.

The role requires MSW or MSS, licensure as CSW/MSW in Delaware, and experience in acute care and maternal-child settings. You will advocate for patients, coordinate care, and respond to crises and complex social needs with

Qualificações

• Master's degree in social work (MSW) or Master of Social Services (MSS) from an accredited graduate school required before hire date.
• Licensed CSW or MSW as required by the state of Delaware prior to hire date.
• Acute Care Social Work experience preferred.
• Maternal-Child position requires experience with neonates, children and families.

Responsabilidades

• Conducts social work psychosocial assessment/reassessment of patients and families.
• Develops and implements intervention plans addressing identified needs.
• Collaborates with the multidisciplinary team to ensure discharge planning.
• Provides counseling and interventions to assist coping with diagnoses, illness, or palliative/terminal care.
• Advocates for patients to obtain necessary services.
• Coordinates family conferences to support care progression.
• Manages cases involving abuse/neglect, domestic violence, guardianship, foster care, and advance directives.
• Works with Care Managers to provide discharge planning services per policies.
