Qualifications

• Bachelor’s degree in health-related field with licensure (Primary Care Case Manager I).
• Master’s degree (Primary Care Case Manager II).
• MHBE Insurance Attester/Counselor Certification within one year of hire.
• Three years of related experience in health care, community, case management, and outreach.

Responsibilities

• Provide social work and medical case management to CBHS patients and families.
• Coordinate referrals and linkage to community resources.
• Assist with Social Security, Medicaid, Medicare and ACA enrollment.
• Document services per CBHC policy and maintain accurate records.
