Job summary

Qualifications

• Bachelor’s degree in health-related field required; licensure as appropriate (Primary Care Case Manager I)
• Master's degree preferred (Primary Care Case Manager II)
• MHBE Insurance Assister/Counselor Certification within one year of hire
• Three years of related experience in a health care setting and community/outreach experience

Responsibilities

• Provide supportive counseling for patients and families.
• Perform comprehensive intake assessments and document duties.
• Coordinate information, caseload priorities, and referrals.
• Follow up on referrals in a timely, accurate manner.
• Review patient needs and care plans with supervisor.
• Maintain accurate data for reporting to funders and partners.
