Job summary

INNOVATIVE INTEGRATED HEALTH INC in Fresno, CA is seeking a Social Work Aide to support the Interdisciplinary Team in our PACE by IIH program. You will provide essential administrative support, assist with scheduling, scribing, and participant notifications to ensure smooth care planning.

You will collaborate with the IDT and eligibility staff, help coordinate with government agencies, and uphold confidentiality while delivering compassionate, participant-centered services to aging adults at

Qualifications

Minimum of one year of experience working with a frail or elderly population.
Experience in a community-based setting or geriatric program preferred.
Excellent written and verbal communication skills.

Responsibilities

Provide administrative support to the Social Work department as needed.
Scribe and contribute to the Interdisciplinary Team (IDT), including scheduling, SDRs, and care planning.
Collaborate with eligibility for ongoing verification and coverage restoration.
Assist with liaison activities between participants and government agencies.
Maintain confidentiality of participant information and follow policies.
