PACE by IIH is seeking an entry-level Social Work Aide to support the Interdisciplinary Team and Social Work department with scheduling, scribing, and participant notifications. The role emphasizes confidentiality and collaboration with government agencies to assist eligibility and care planning.

Candidates should have a high school diploma, CPR/First Aid, and a valid California driver’s license. The position may require driving and flexible hours, with comprehensive benefits including 401(k)

Qualifikationen

• Experience working in an interdisciplinary team.
• Strong communication, written and verbal.
• Proficiency with EHR and MS Office.
• Must be able to meet confidentiality requirements.

Aufgaben

• Provide administrative support to the Social Work department as needed.
• Scribe and support IDT activities including scheduling, SDRs, and care planning.
• Collaborate with eligibility for ongoing verification and coverage restoration.
• Serve as liaison between participants and agencies (Aging, SSA, Medicaid, etc.).
• Maintain confidentiality of participant information; attend meetings and trainings.
• Follow center policies and adhere to attendance and break schedules.
• Be flexible with hours and may drive as needed.
