Job summary

Staten Island Care Center in Staten Island, NY is seeking an experienced Social Services Worker to support residents and families in a compassionate, professional manner. You will conduct assessments, update care plans, coordinate discharges, and assist with Medicare/Medicaid and community resources, working closely with nursing and medical staff.

Qualifications

• 1-2 years of social services experience required.
• Bachelor's or master's Degree required.
• Long-term care, skilled nursing, rehabilitation, or healthcare experience preferred.
• Knowledge of care planning, discharge planning, MDS, Medicare/Medicaid, and community resources.
• Strong communication, organization, and documentation skills.
• Compassionate, professional, dependable, and highly organized.
• Strong problem-solving skills and ability to work effectively as part of an interdisciplinary team.
• Experience with EMR systems preferred.

Responsibilities

• Provide social, emotional, and psychosocial support to residents and families.
• Complete assessments, social histories, progress notes, and care plans.
• Participate in care plan meetings, MDS, discharge planning, and interdisciplinary team meetings.
• Coordinate discharges, transportation, referrals, and community resources.
• Assist residents and families with Medicare, Medicaid, financial assistance, and community services.
• Review and coordinate MOLST/POLST and advance directive documentation.
• Advocate for resident rights, preferences, and quality of life.
• Maintain accurate documentation and ensure compliance with applicable regulations.
• Collaborate closely with nursing, medical, rehabilitation, and other departments.
• Participate in quality assurance activities, facility surveys, and departmental initiatives.
• Build strong relationships with residents, families, staff, physicians, and community agencies.

Salary: $60,000 – $65,000 per year
