Job summary

Qualifications

• Licensed Social Worker in the State of Alabama preferred.
• Degree or educational background in Social Work, Social Sciences, or a related field.
• Previous long-term care or skilled nursing experience preferred.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal skills.
• Ability to effectively follow up with residents, families, staff, and outside providers.

Responsibilities

• Plan, organize, implement, and evaluate the facility’s Social Services program.
• Advocate for residents and serve as a liaison between residents, families, staff, and outside resources.
• Identify and address residents’ social, emotional, and psychosocial needs.
• Assist residents and families with adjustment to long-term care and changes in health or level of care.
• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
• Participate in resident care planning and interdisciplinary team meetings.
• Assist with discharge planning and coordination of community resources when applicable.
• Communicate regularly with residents and families regarding needs, concerns, and available resources.
• Ensure Social Services are provided in accordance with applicable federal, state, and local regulations.
• Work closely with facility leadership and the interdisciplinary team to promote each resident’s overall quality of life.
