Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, or related field.
• Minimum 2 years of social work experience.
• Experience in a skilled nursing facility preferred.
• Strong communication and interpersonal skills.
• Knowledge of long-term care regulations and discharge planning is a plus.

Responsibilities

• Perform psychosocial assessments for residents at admission and during stay.
• Provide emotional support and counseling to residents and families.
• Participate in care plan meetings with the interdisciplinary team.
• Assist with discharge planning and community resource coordination.
• Help navigate healthcare services and resident benefits.
• Advocate for resident rights and satisfaction.
• Maintain accurate documentation per policies and regs.
• Support resident adjustment and transition needs.
