Job summary

Ohana Pacific Health in Honolulu, HI is seeking a Social Services Manager to plan, coordinate and implement the emotional and social needs program for residents. You will ensure residents' rights are communicated and provide counseling and case management to residents and families.

You will also advise the team and external partners to solve residents' needs through development of social service programs, ensuring compassionate, holistic care in a post-acute setting.

Qualifications

• Bachelor's Degree in Social Work or combination of education and experience.
• Experience in a health care setting preferred, supervisory experience preferred.
• Licensed by the State of Hawaii as a Social Worker.

Responsibilities

• Plans, coordinates and implements the overall emotional and social needs program for residents.
• Provides individual and family counseling and case management services to residents and families when necessary.
• Provides consultation to team members, community entities and agencies to address residents' needs through social service programs.

The Position:

We're looking for a passionate Social Services Manager to plan, coordinate and implement the overall emotional and social needs program for residents. This position encompasses:

• Assures that all residents and families are informed fully concerning "residents rights"
• Provides individual and family counseling and case management services to individuals and families when necessary.
• Provides consultation to members of the team, community, agencies, etc. in efforts to solve the needs and problems of the residents through the development of social service programs.

Requirements:
