Job summary

Ohana Pacific Health is seeking a Social Services Manager to lead the emotional and social needs program for residents in Hawaii. You will coordinate services, ensure residents and families are informed about rights, and provide counseling or case management as needed.

The role includes consulting with the care team and community agencies to develop social service programs addressing resident needs and rights within a compassionate, mission-driven culture.

Qualifications

• Master's Degree in Social Work or combination of education and experience.
• Experience in a health care setting preferred, supervisory experience preferred.
• Licensed by the State of Hawaii as a Social Worker.

Responsibilities

• Plans, coordinates and implements the emotional and social needs program for residents.
• Ensures residents and families are informed about residents' rights.
• Provides counseling and case management; consults with team to solve resident needs.

The Position:

We're looking for a passionate Social Services Manager to plan, coordinate and implement the overall emotional and social needs program for residents. This position encompasses:

• Assures that all residents and families are informed fully concerning "residents rights"
• Provides individual and family counseling and case management services to individuals and families when necessary.
• Provides consultation to members of the team, community, agencies, etc. in efforts to solve the needs and problems of the residents through the development of social service programs.

Requirements:

• Master's Degree in Social Work or combination of education and experience.
• Experience in a health care setting preferred, supervisory experience preferred.
• Licensed by the State of Hawaii as a Social Worker
