Regency Pacific is seeking a Social Services Liaison to support residents' social, emotional, and psychological well-being in a skilled nursing setting. You will plan discharges and help residents reach their full potential.

You will coordinate social services with the management team, document in clinical records, and engage families in care planning while maintaining residents' rights and confidentiality standards. A Bachelor's or Master's in Social Work is required.

Qualifications

• Bachelor's or Master's degree in Social Work.
• Skilled Nursing experience required.
• 1–2 years in geriatrics/long-term care preferred.

Monday - Friday / 40 Hours Per Week!

Must Have Skilled Nursing Experience!

As Social Services Liaison, you assist in providing for residents' social, emotional, and psychological needs. You plan for residents' discharge and ensure they reach their full potential.

You will:

• Support individual needs, preferences, routines, concerns, and choices
• Arrange needed adaptive equipment, clothing, personal items, and referrals to outside agencies
• Coordinate social services with the management team
• Document in the patient's clinical record
• Communicate with and encourage the family to participate in care planning
• Provide residents with information concerning resident rights, living wills, etc. Explain as necessary
• Report to community Administrator

You currently:

• Promote resident rights, dignity, and confidentiality standards
• Problem-solve creatively, are organized, and flexible.
• Display excellent written and oral communication skills
• Prefer one to two years of working in geriatrics/long-term care.
• Must have Bachelor's or Master's degreein Social Work.
