Job summary

Washington Care Center in Seattle, WA is seeking a part-time Social Services Assistant to support the Social Services Director in addressing residents’ social, emotional, and psychological needs and assist in discharge planning.

Duties include meeting residents and families on advance directives, evaluating needs, coordinating services, assisting with financial planning, and keeping families informed of changes and status. Strong communication and a background in geriatrics are required.

Qualifications

• Associate degree required; meets state certification requirements.
• 1–2 years of experience in geriatrics.
• Preferred: Experience in Behavior Add-on Programs.

Responsibilities

• Meet with resident and family to review policies on advance directives, resident rights and non-discrimination.
• Conduct and document a social services evaluation, identifying resident problems/needs.
• Provide or arrange social work services as indicated by resident/family needs.
• Assist resident with financial planning as needed.
• Keep the resident’s family informed of problems, transfers, and changes of level assignment.
• Meet with facility staff on resident issues and participate in discharge planning.
• Maintain confidentiality of all resident care information.
• Review complaints and grievances and report actions to the Administrator.
• Participate in resident/group council meetings and provide support services to such councils.

Job Summary

Washington Care CenterinSeattle WA is seeking aSocial Services Assistantto help the Social Services Director in identifying and providing for each resident’s social, emotional, and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for the resident’s discharge.

Essential Functions

• As part of the admission process, meet with the resident and family to review policies on advance directives, resident rights and non-discrimination.
• Conduct and document a social services evaluation, including identification of resident problems/needs.
• Provide/arrange for social work services as indicated by resident/family needs.
• Assist resident, as needed, with financial planning.
• Keep the resident’s family informed of resident problems, personal needs, transfers, and changes of level assignment.
• Meet with the appropriate facility staff concerning resident issues.
• Act in compliance with all corporate, state, federal, and other regulatory standards.
• Access continuing education opportunities appropriate to discipline and responsibilities.
• Assist with discharge planning.
• Works under the direction of the Social Services Director.
• Maintain the confidentiality of all resident care information including protected health information.
• Report known or suspected incidents of unauthorized disclosure of such information.
• Review complaints and grievances made by the resident and make a written/oral report to the Administrator indicating what action(s) were taken to resolve the complaint or grievance. Follow facility’s established procedures.
• Provide residents with information concerning resident rights, living wills, etc. Explain as necessary.
• Participate in resident/group council meetings as requested and provide support services to such council.

Education, Licensure, & Experience Requirements

• Must have a minimum of an Associate’s Degree and meets State certification requirements.
• A minimum of 1 to 2 years working in geriatrics.
• Preferred: Experience in Behavior Add-on Programs.
