The role requires identifying interventions, developing care plans, and maintaining collaboration with medical professionals and facilities to support residents and families in a long-term care setting.

Qualifications

• Two years of job-related experience, preferably in long-term care.
• Bachelor’s degree in social work or related field preferred.
• Experience with social work, preferably working directly with patients and their family members at a nursing facility.
• Detail oriented, computer literate and possesses excellent communication skills.
