Job summary

Fundamental seeks a licensed clinical social worker director to plan, develop, organize, implement, evaluate and direct the facility's social service programs, ensuring high-quality patient care and regulatory compliance.

The position requires a bachelor's degree in social work or a human services field, at least one year of relevant experience in a medical facility, and strong communication, leadership and organizational skills; licensure as a clinical social worker is required.

Qualifications

• Bachelor's degree in social work or a human services field (including sociology, gerontology, special education, rehabilitation counseling, psychology) required.
• Minimum of 1 year experience in a human services field in a hospital, skilled nursing facility, or other medical facility.
• Must be able to organize and prioritize many tasks effectively; strong communication and leadership skills expected.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct the facility's social service programs.
• Develop and implement policies for identifying medically-related social and emotional needs of patients/residents.
• Assist in discharge planning, seek financial assistance, and refer to community agencies when specialized help is required.

Position Summary

The Social Services Director is responsible for planning, developing, organizing, implementing, evaluating and directing of the social service programs of the facility

Posted Salary Range

USD $29.00 - USD $33.00 /Hr.

Wage Commensuration

The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.

Duties & Responsibilities

• Responsible for planning, developing, organizing, implementing, evaluating and directing of the social service programs of the facility
• Develops and implement policies and procedures for identifying the medically-related social and emotional needs of the patients/residents
• Assists in providing solutions to social and practical environmental problems, including seeking financial assistance, discharge planning (including collaboration with community agencies) and providing referrals to other community agencies when specialized assistance is required

Qualifications & Requirements

• An individual with a minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to sociology, gerontology, special education, rehabilitation counseling, and psychology.
• Must have, as a minimum, one (1) year experience in a human services field in a hospital, skilled nursing care facility or other related medical facility
• Must be able to organize and prioritize many tasks effectively
• Ideal candidate must be a skilled communicator, director and motivator
• Reliable transportation required
