Qualifications

• BS/LSW/MSW degree required.

Responsabilités

• Identify the social, emotional, and financial needs of residents and families.
• Oversee the daily functioning of the Social Service Designee.
• Assist residents and families with adjustment, problem solving, and meeting social, emotional, and financial needs.
• Coordinate discharge planning and arrange for community services/referrals.
• Advocate for residents and families regarding resident rights.
• Educate staff on residents' emotional and social needs.
