Job summary

STGi seeks a qualified social services professional for a forthcoming opening in Salisbury, NC. The role plans, develops, organizes, implements, evaluates and directs the facility's social service programs to support residents and families.

Responsibilities include interviewing residents or families, guiding Medicare/Medicaid and financial assistance options, referring to appropriate agencies, coordinating with other departments, supporting QA/PI activities, and ensuring compliance with state

Qualifications

• Experience in social services within healthcare or residential care.
• Knowledge of Medicare/Medicaid programs and financial assistance.
• Ability to lead and supervise staff and coordinate with departments.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct the social service programs of the facility.
• Interview residents or families to obtain social history.
• Inform residents/families about Medicare/Medicaid and other financial assistance.
• Refer residents/families to appropriate social service agencies when needed.
• Coordinate social services with other departments as necessary.
• Attend QAPI meetings and help implement action plans to address deficiencies.
• Keep current with federal and state regulations and advise on policy changes to the Administrator.
• Demonstrate the ability to make independent decisions when warranted.
• Provide care appropriate to the age-related needs of residents.
• Be a supportive team member and promote teamwork and a collaborative approach.
