Job summary

Qualifications

• All facilities must provide medically related social services to residents per State and Federal rules.
• Social Services Designee must have a bachelor’s degree in social work or related human services field.
• One year of supervised social work experience in a health care setting.

Responsibilities

• Oversee development, implementation, supervision and ongoing evaluation of the Social Services Department.
• Direct/delegate social services components of the care plan and assessments.
• Manage preadmission services and discharge planning with community-based follow-up.
• Establish departmental QA procedures and adapt as needed.
