Job summary

Journey is seeking a Social Services Director to lead the development, implementation, supervision and ongoing evaluation of the Social Services Department, aimed at helping residents achieve and maintain their highest practicable well-being. You will work with a dedicated home office team to support community-based outcomes and ensure regulatory conformity.

You will guide the care team in discharge planning, QA processes, and preadmission services, fostering strong partnerships with residents

Qualifications

• Bachelor’s degree in social work or related field is required.
• One year of supervised social work experience in a health care setting.
• Licensed Social Worker in the required state.

Responsibilities

• Identify the need for medically related social services and ensure services meet state and federal regulations.
• Complete or direct the social services component of the comprehensive assessment and contribute to the care plan.
• Direct the Social Services department including overseeing preadmission services for inbound residents.
• Facilitate residents’ safe transition back into the community through discharge planning and coordination of community-based services.
• Oversee establishment and modification of departmental QA procedures.

Responsibilities

• Identifying the need for medically related social services and ensuring that these services are provided in accordance with State and Federal regulations.
• The Social Services Director will complete and/or direct/delegate the completion of the social services component of the comprehensive assessment. The Social Services Director will also contribute to and/or direct/delegate contribution of social services goals and approaches to the care plan.
• Directing the Social Services department includes overseeing preadmission services for inbound residents
• The Social Services Director will facilitate residents’ safe transition back into the community through interdisciplinary discharge planning and arrangement of community-based services and follow-up care.
• The Social Services Director is responsible for overseeing the establishment of departmental QA procedures and modification of those procedures where appropriate.

Required Qualifications

All facilities must provide medically related social services to residents. It is not required that a qualified social worker necessarily provide all of these services, except as required by State law. The regulations do not require a Social Worker when a facility has equal to or less than 120 beds.

• A minimum of a bachelor’s degree in social work or another human services field to include, but not limited to, sociology, gerontology, special education, rehabilitation counseling, or psychology.
• One year of supervised social work experience in a health care setting working directly with individuals.
• License Social Worker License in required state.
