Job summary

Wheatridge Manor Care Center in Wheat Ridge, CO, seeks a Director of Social Services to lead the department and support residents and families through placement, illness, and care planning, in accordance with standards and the company’s policies.

You will supervise social services staff, perform psychosocial assessments, and coordinate with the interdisciplinary team to ensure dignity, respect, and high-quality, person-centered care for every resident.

Qualifications

• 2-3 years' related experience; supervisory experience preferred.
• Experience working in a skilled nursing facility.
• Bachelor's degree in social work or related field; BA acceptable in smaller facilities.
• Commitment to high-quality, person-centered care with dignity, respect, compassion and integrity.
• Ability to interact professionally with residents, families, staff, and external agencies.
• Ability to relate to residents with varying cognitive/functional levels.

Responsibilities

• Respects and upholds residents' rights, including knocking before entering rooms.
• Complies with company and departmental policies and procedures.
• Assesses residents on admission, quarterly, and with changes in condition for social service needs.
• Develops and maintains plan of care with facility interdisciplinary team.
• Assists residents and families with transition and adjustment to placement in the facility.
• Determines legal status on admission and ensures advance directives are respected; supports DNR decisions.
