Job summary

San Bruno Skilled Nursing is seeking a compassionate Social Services Director to advocate for residents’ social, emotional, and psychosocial well‑being and to collaborate with families, healthcare providers, and community resources. The role includes coordinating services to ensure high‑quality care and support for residents and families.

The director will lead assessment, care planning, and discharge coordination while maintaining documentation and compliance.

Qualifications

• Bachelor's degree in Social Work, Human Services, or related field required.
• Minimum of 2 years of supervised social work experience in healthcare.
• MSW preferred.
• Skilled nursing or long‑term care experience preferred.
• Experience with PointClickCare (PCC) preferred.
• Excellent communication, organizational, problem‑solving, and interpersonal skills.
• Ability to maintain confidentiality and exercise professional judgment.

Responsibilities

• Assess residents' social, emotional, and family needs and develop individualized care plans.
• Assist residents in achieving the highest practicable level of independence and well‑being.
• Provide medically related social services to support residents' physical, mental, and psychosocial health.
• Offer counseling and emotional support to residents and families coping with illness, disability, grief, and life transitions.
• Coordinate community resources and agency referrals to meet resident needs.
• Assist residents and families with healthcare decisions and discharge planning.
• Coordinate outpatient appointments and ancillary services, including dental, optometry, podiatry, and psychiatric care.
• Maintain accurate and timely social services documentation.
• Collaborate with interdisciplinary team members to ensure comprehensive resident care.
• Assist with resident belongings management and related concerns.
• Provide staff education and in‑service training as needed.
• Foster positive relationships with residents, families, staff, and community partners.
